Instructions to Financial Institutions

Financial Code 4053 (d) (2) (E): "If a financial institution uses a form other than that set forth in this subdivision, that form shall be filed with the Office of Privacy Protection within 30 days after it is first used."

1. Financial institutions filing such alternate notices with the Office of Privacy Protection should submit them in Portable Document Format (PDF).
2. Send them to the following e-mail address: privacy@dca.ca.gov.
3. Please write "SB 1 Notice" in the subject line.1

The notices filed will be posted on the Office of Privacy Protection web site at www.oispp.ca.gov/consumer_privacy/sb1/sb1notices.asp.

Note: The Office of Privacy Protection does not approve the notices. Financial institutions seeking approval of their alternate notices should contact their State financial regulator, if they have one, or the Attorney General's Office.
